If you are a Public Health and Preventive Medicine or Community Medicine Specialist, or are a physician working in the field of public health and preventive medicine, you are entitled to submit an application for membership here.

If you would prefer to submit your membership form in hard copy, you can request form by email (info@phpc-mspc.ca) or print a Membership Application form and return by mail or fax.
